We are looking for a dedicated Home Health Registered Nurse (RN) for a contract opportunity. This role involves minimal travel as visits will be conducted within the Assisted Living Facility (ALF) setting.
Position SummaryThe RN will provide home-based care to patients by utilizing the nursing process, which includes assessment, planning, intervention, implementation, and evaluation, in alignment with established care philosophies. Home Health nurses are essential in delivering skilled healthcare to independent patients, facilitating their recovery post-surgery or during illness.
Key Responsibilities- Connect patients with community resources to assist with referrals and address social service needs.
- Monitor and support patients who seek services outside the practice, such as Emergency Departments and Hospitals.
- Conduct follow-ups with patients shortly after an emergency room visit or hospital discharge.
- Communicate test results and care plans to patients and their families under the guidance of practitioners.
- Provide self-management support for patients with chronic conditions as identified by clinical teams.
- Graduate of an accredited nursing program.
- Current nursing license in good standing with the State licensing board.
- At least one (1) year of experience in Home Health.
- Mileage or drive time compensation when working between facilities.
- Guaranteed 40-hour work week.
