The Clinical Liaison plays a crucial role in ensuring effective patient placement through comprehensive clinical evaluations. This position involves close collaboration with the Case Management team and the Medical Director to facilitate smooth admissions and address patient requirements.
Key Responsibilities:
- Conduct thorough clinical assessments to guarantee timely patient placements.
- Work in partnership with Case Management and the Medical Director to optimize patient care.
- Oversee the admission process and effectively communicate patient needs to relevant stakeholders.
- Serve as an advocate for the facility within various healthcare systems.
Qualifications:
- 1-2 years of experience in acute inpatient rehabilitation settings.
- Familiarity with inpatient rehabilitation protocols and standards.
- Competence in reviewing medical records to determine rehabilitation necessity.
- Exceptional organizational and communication abilities.
- Current licensure in the relevant field in Florida.
- BLS Certification is required.
- CARE Tool Credentialing is preferred when applicable.
Work Schedule: Saturday and Sunday, with 8-hour shifts.
