About the Role:

We are seeking a skilled Clinical Care Manager to join our team at LifeBridge Health. As a Clinical Care Manager, you will play a critical role in managing individualized patient-centered care through the use of evidence-based medicine and data analytics.

Key Responsibilities:

  • Manage Patient Care: Utilize the nursing process and primary nursing principles to provide comprehensive care management for patient populations, with the ultimate goal of improved health outcomes, reduced healthcare costs, and better coordination of care.
  • Accept Care Management Referrals: Work closely with interdisciplinary teams to accept referrals and maintain assigned caseloads, applying critical thinking skills and leadership abilities to meet patients' and families' needs.
  • Coordinate Care Transitions: Facilitate timely access to appointments and services required by patients, ensuring seamless transitions between care settings.
  • Evaluate Effectiveness: Regularly evaluate the effectiveness of care plans with IDT essential job functions, making adjustments as necessary to ensure optimal patient outcomes.

Requirements:

  • Education: Graduate of an accredited school of nursing, with a Bachelor's degree required; Master's degree preferred.
  • Licensure: Current/valid registered nurse license in the state of Maryland (or compact state as applicable).
  • Experience: 3-5 years demonstrated proficiency in acute care nursing, with knowledge and skills in care management and population health.

Competitive Salary: $48,192 - $72,281 per year, based on experience.
