Home Health LPN Job Description

PruittHealth @Home is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team. As an LPN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.

Responsibilities:
  • Conduct comprehensive assessments and evaluations of patients to identify their needs and develop individualized care plans
  • Administer medications and treatments as prescribed by physicians, and provide education to patients and their families on medication management and self-care
  • Provide wound care, ostomy care, and other specialized care as needed
  • Collaborate with the interdisciplinary team to coordinate care and services, and communicate effectively with patients, families, and healthcare providers
  • Document patient information accurately and thoroughly, and maintain confidentiality of patient records
Requirements:
  • Current LPN licensure in the state of employment
  • Minimum of 1 year of experience in home health or a related field
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills
  • Ability to lift, push, and pull up to 25 pounds, and stand for long periods of time
