Adult Nurse Practitioner

Americares Foundation is seeking an experienced Adult Nurse Practitioner to join our team in Norwalk, CT. As a key member of our medical team, you will provide high-quality primary care to adults with chronic conditions, working collaboratively with our Clinic Director and administrative support.
Key Responsibilities- Provide thorough and appropriate medical exams, rendering proper care as required in the diagnosis and treatment of patients within the limits of experience and professional training.
- Use Athena Electronic Health Record to document examination, diagnosis, and treatment notes, providing clear orders and being available to nursing staff for direction, support, and clarification of the medical plan of care.
- Appropriately utilize diagnostic and laboratory services as required for quality care, identifying patients beyond our scope of care, making patient referrals to appropriate specialty services, and determining medication needs utilizing available resources.
- Provide care to patients according to the primary medical home model, following preventative care guidelines and participating in quality improvement activities.
- Serve as a consultant with regard to decisions pertaining to medical policy and patient care, and performance standards.
- Master's degree and license from an accredited nurse practitioner program with certification in adult medicine or family practice and the ability to work independently in the State of Connecticut.
- At least two years of experience as a NP providing care to adults with chronic disease.
- Experience working in primary care settings with an adult patient population.
- Experience in supporting patients with chronic healthcare conditions.
- Demonstrated ability to maintain current knowledge of treatment protocols.
- Excellent communication skills, including verbal and written, and the ability to give and receive feedback.
- Excellent bedside manner and patient support.
- Experience and ability to work with a medical interpreter, as needed.
