Responsibilities:
- Coordinates the total Plan of Care for patients in the Home Care setting
- Assesses, plans, implements, and evaluates patient care to ensure the best possible outcomes
- Provides leadership for the Health Care team by working cooperatively with other team members
Requirements:
- AAS in Nursing required; BSN preferred
- 1 year of psychiatric experience as a RN with good working knowledge of psychiatric diagnosis, psychiatric medication, and treatment (within the last 3 years) required
- 1 year of Medical/Surgical experience as a RN required
- RN license, NYS Driver's license, and current APNA Transitions in Practice Certificate Required upon hire
