We are seeking a skilled nursing professional to join our team as a Home Health Registered Nurse. This role offers the opportunity to make a meaningful difference in patients' lives while working in a dynamic and supportive environment.

Key Responsibilities:
  • Develop and implement patient care plans in collaboration with physicians and other healthcare professionals
  • Provide admission, case management, and follow-up visits for home health patients
  • Supervise and coordinate care with home health aides and external providers
  • Maintain accurate and timely documentation of patient progress and care plans
Requirements:
  • Diploma, Associate, or Bachelor's degree in Nursing
  • Minimum one year of nursing experience, preferably in home health or similar settings
  • Current and unrestricted RN licensure
  • CPR certification
