Role Overview:
In this position, you will be instrumental in delivering recovery-oriented services to adults (18 and older) facing serious and persistent mental health challenges. Our services are designed for individuals eligible for Medi-Cal in the Orange County area.

Key Responsibilities:
- Conduct comprehensive evaluations and assessments of clients' situations, needs, and challenges, collaboratively developing detailed care plans with client involvement.
- Oversee service coordination, ensuring clients effectively implement their care plans by facilitating access to community resources and monitoring the effectiveness of support services.
- Maintain meticulous records of daily activities, including service progress notes and billing documentation, adhering to company and governmental standards.
- Provide outreach and information referral services to individuals who may not qualify for case management services.
- Uphold HIPAA regulations, ethical standards, and professional boundaries in all interactions with clients and staff.
- Participate in team meetings, training sessions, and other required activities to enhance service delivery.

Qualifications:
- A degree in human services, psychology, or a related field is preferred, along with at least one year of relevant experience in behavioral health.
- A minimum of two years of prior case management experience is required, with established connections to community resources being advantageous.

Additional Information:
- The role may involve transportation of clients, requiring the use of a personal vehicle.
- Participation in on-call duties and after-hours responsibilities may be necessary as assigned.
