Job Summary

We are seeking a skilled Nursing Professional to join our team at IntelyCare. As a Licensed Practical Nurse (LPN), you will provide high-quality patient care in post-acute facilities, working closely with our team to ensure the best possible outcomes.

Key Responsibilities
  • Provide compassionate nursing care to residents, monitoring their physical and mental conditions and tracking vital signs.
  • Perform resident intakes, documenting medical histories and health concerns, and administering medications as needed.
  • Cultivate a comfortable and safe facility environment, working with staff to maintain a clean and organized space.
  • Track residents' medical records and charts, ensuring accurate and up-to-date information.
  • Assist with the setup of IVs, immunizations, and phlebotomy procedures, and maintain a proper inventory of medical supplies.
  • Work with staff to orient and train new colleagues, promoting a positive and supportive team environment.

Requirements
  • Licensed Practical Nurse (LPN) certification.
