Job Title: Travel Nurse RN Case Manager

Tact Staff is seeking a skilled Travel Nurse RN Case Manager to join our team in Gainesville, Florida.

Job Summary:

We are looking for a highly motivated and experienced RN Case Manager to work in our Acute Care department. As a Travel Nurse RN Case Manager, you will be responsible for providing high-quality patient care and managing case loads in a fast-paced hospital setting.

Responsibilities:
  • Provide direct patient care and manage case loads in the Acute Care department
  • Collaborate with interdisciplinary teams to develop and implement patient care plans
  • Conduct thorough assessments and develop effective care plans to meet patient needs
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
  • Manage and coordinate patient care services to ensure timely and efficient delivery of care
Requirements:
  • Current RN licensure in the state of Florida
  • Minimum 2 years of experience in Acute Care Case Management
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Proficiency in electronic medical records and other healthcare software
