Clinical Diabetes Education Specialist

**About the Role:**The Clinical Diabetes Education Specialist is a crucial position within the Veterans Affairs, Veterans Health Administration, playing a vital role in delivering comprehensive care to veterans. This specialist will work closely with patients, families, and healthcare teams to provide education and support on diabetes management.
**Responsibilities:**
* Develop and implement individualized diabetes education plans tailored to patients' needs and goals.
* Collaborate with multidisciplinary teams to coordinate patient care and ensure seamless transitions between healthcare services.
* Provide ongoing education and support to patients and their families, promoting self-management skills and empowering them to make informed decisions about their care.
* Conduct learning needs assessments, develop educational materials, and facilitate group and individual sessions to enhance patient knowledge and skills.
* Participate in quality improvement initiatives, contributing to the development and implementation of best practices in diabetes care.
* Foster strong relationships with community partners, healthcare providers, and stakeholders to ensure optimal patient outcomes and access to resources.
**Requirements:**
* Current licensure as a registered nurse (RN) or certification as a Certified Diabetes Educator (CDE).
* A minimum of 2 years of experience in clinical nursing, with a focus on diabetes education and management.
* Strong communication and interpersonal skills, with the ability to work effectively with diverse populations and healthcare teams.
* Proficiency in electronic health records (EHRs) and other relevant software systems.
* Ability to travel to different locations within the VA network, as needed.
