Job Title: Registered Nurse - Hospice Liaison

VNA Health Group is seeking a skilled Registered Nurse to join our dedicated Hospice Team in Monmouth County. As a Hospice Liaison, you will play a vital role in assessing patients for hospice needs and coordinating total care for hospice patients and their families.

Responsibilities:
  • Assess patients for hospice needs and provide counseling to patients and their families to promote continuity of care.
  • Establish and maintain liaison with physicians, external and internal customers to ensure seamless care coordination.
  • Manage and monitor the quality and appropriateness of services and programs to ensure high-quality care.
  • Conduct community outreach activities to improve community access to hospice care.
  • Develop and implement a discharge program at Centrastate Medical Center and Monmouth Medical Center to ensure smooth transitions of care.
  • Adhere to agency infection control and safety policies to ensure a safe work environment.
  • Adhere to the organization's policy in regards to absenteeism and appearance to maintain a professional work environment.
Qualifications:
  • Bachelor's degree in Nursing required.
  • Excellent clinical skills and experience in hospice nursing and community health nursing.
  • Excellent verbal and written communication skills to effectively communicate with patients, families, and healthcare teams.
  • Excellent customer service, organizational, and interpersonal skills to provide high-quality care and support.
  • Strong computer skills with proficiency in MS Office Suite to efficiently manage patient data and care coordination.
