Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a PCM Evaluation Department Nurse. As a key member of our community care team, you will be responsible for conducting evaluations and reevaluations of members who are applying for personal care services in accordance with MassHealth regulations.

Key Responsibilities
  • Perform initial evaluations and annual reevaluations of members in accordance with MassHealth regulations
  • Submit completed evaluations within the specified timeframe
  • Respond to hours adjustment requests in a timely manner
  • Communicate with PCA Eval Dept Assistants regarding no shows, no contacts, hospitalizations, termination requests, and other concerns
  • Document progress notes in the database in a timely manner
  • Work collaboratively with other members of the PCA Program
  • Represent the agency in a professional, positive manner and provide services that are culturally sensitive
Requirements
  • Current Massachusetts license to practice as a Registered Nurse in good standing with the Board
  • 2 years recent experience working with direct care of the elderly or disabled population
  • Ability to establish and maintain professional relationships with consumers, family members, co-workers, and community professionals
  • Ability to exercise sound judgment in making decisions
  • Ability to independently organize work and meet deadlines
Preferred Qualifications
  • Bachelor's Degree
  • Familiarity with MassHealth PCA Program
  • Fluency in other languages
Work Environment

This role operates in a professional office and home office environment. The employee is frequently required to sit, stand, bend, stoop, walk, carry, and lift objects.

Travel

Travel within the coverage area is required for this position. The employee must have a valid driver's license and reliable transportation.
