We are seeking a highly motivated and experienced RN to join our team as a Hospice Patient Care Coordinator. In this role, you will be responsible for collaborating with the Director of Clinical Services to develop and implement processes that ensure efficient and effective patient outcomes.
The ideal candidate will have two years of experience as a Hospice RN, with at least one year of supervisory experience. A graduate degree from an accredited school of nursing and an active license to practice as an RN in the state of employment are also required.
You will be responsible for supervising, evaluating, motivating, and developing professional and paraprofessional employees. You will also use clinical decision making to provide oversight and guidance to the healthcare team to efficiently manage the assigned patient population.
