About the RoleAs a Home Health Physical Therapist, you will use your clinical skills and expertise to provide high-quality care to patients in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Conduct thorough assessments of patients' physical abilities and develop personalized care plans to improve their mobility, strength, and overall well-being.
- Provide manual therapeutic exercises, gait training, balance, and other interventions to help patients achieve their goals.
- Continuously assess and revise care plans to ensure optimal outcomes and patient satisfaction.
- Collaborate with our nursing team to provide comprehensive care and support to patients and their families.
- Active and current license to practice as a professional Physical Therapist (PT) in the state of New Jersey.
- Previous home healthcare experience is welcomed, but not required. We will provide comprehensive training to ensure your success.
- Ability to travel to cases as assigned.
- A calm and reassuring presence, active listening skills, excellent assessment skills, and strong written and verbal communication.
