Job Summary

Eden Health is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality care to our patients in the comfort of their own homes. As a Hospice LPN, you will play a vital role in delivering compassionate and personalized care to individuals with life-limiting illnesses.

Key Responsibilities
  • Provide skilled nursing care to patients in accordance with their individualized plans of care.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and effective communication.
  • Assess patient needs and develop education plans to promote optimal health outcomes.
  • Participate in quality improvement initiatives to enhance patient care and satisfaction.
  • Document patient interactions and care provided in a timely and accurate manner.
Requirements
  • Graduate of an accredited nursing program.
  • Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of practice.
  • Experience in hospice or home health care preferred.
  • Valid driver's license and reliable transportation.
  • Proof of automobile insurance and a clean driving record.
  • Current CPR certification.
  • Able to pass a criminal background check and national sex offender clearance.
