About the Role:

As an Ambulatory Care Coordinator, you will play a vital role in providing high-quality patient care and promoting cost-effective utilization of healthcare resources in the outpatient setting.

Main Responsibilities:

  • Facilitate care coordination with patients, families, caregivers, and healthcare teams to identify needs, preferences, and available resources.
  • Determine appropriate levels of care and coordinate interdisciplinary care delivery in collaboration with medical providers.
  • Triage phone calls, respond to patient and family inquiries, and communicate effectively with healthcare teams.
  • Advocate for patients and families, ensuring access to services and insurance benefits while protecting their health, safety, and rights.
  • Evaluate options, balancing cost and quality to achieve optimal clinical and financial outcomes.
  • Assess patient outcomes affected by care management processes and identify opportunities for improvement.
  • Support multidisciplinary teams in care delivery and patient education, adhering to population-specific guidelines.

Requirements:

  • A registered nurse graduate from an accredited school with current state licensure.
  • At least one year of experience in an ambulatory healthcare facility or hospital.
  • BLS certification and proficiency in using personal computers and electronic medical records.
