Job Summary

We are seeking a highly skilled and experienced Outpatient Pharmacist to join our team at Soliant Health. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and optimizing medication therapy to improve patient outcomes.

Key Responsibilities
  • Provide Comprehensive Medication Management Services: Develop and implement individualized medication plans to ensure patients receive the best possible care.
  • Collaborate with Healthcare Providers: Work closely with physicians and other healthcare professionals to optimize medication therapy and improve patient outcomes.
  • Educate Patients: Provide patients with clear and concise information about their medications, including side effects, dosages, and interactions.
  • Monitor Patient Safety: Identify and mitigate potential medication-related risks and adverse reactions.
  • Ensure Regulatory Compliance: Maintain accurate records and ensure compliance with all relevant laws and regulations.
Requirements
  • Active Pharmacist License: Must be licensed to practice pharmacy in the state of Utah.
  • Experience: At least 2 years of experience as an outpatient pharmacist.
  • Clinical Knowledge: Strong clinical knowledge and expertise in pharmacotherapy.
  • Communication Skills: Excellent communication and interpersonal skills.
  • Teamwork: Ability to work independently and as part of a team.
  • Continuous Learning: Commitment to ongoing learning and professional development.
