We are seeking a highly skilled Virtual Registered Nurse to join our team as a Patient Care Coordinator. Key Responsibilities- Assess, plan, implement, coordinate, monitor, and evaluate patient plan of care from admission to discharge
- Utilize electronic medical records (EMRs) to document patient information and coordinate care
- Collaborate with healthcare teams to ensure effective and efficient patient care
- Develop and implement patient education plans to promote health and wellness
- Participate in quality improvement initiatives to enhance patient outcomes
- Licensed as a Registered Nurse in the State of Indiana
- Bachelor of Science in Nursing preferred
- Graduate of an accredited school of nursing
- Certification by a nationally recognized nursing organization is preferred
- Basic Life Support for Healthcare Providers certification
- Minimum ten (10) years of experience in health condition management, including adult condition management, and electronic medical record (EMR) experience
