Join Our Team as a Weekend Home Health Physical Therapist

We are seeking a compassionate and skilled Physical Therapist to join our team as a Weekend Home Health Physical Therapist. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role

As a Weekend Home Health Physical Therapist, you will be responsible for providing high-quality physical therapy services to patients in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities
  • Conduct thorough assessments of patients' physical abilities and develop personalized care plans to address their needs.
  • Provide manual therapeutic exercises, gait training, balance, and other interventions to help patients achieve their goals.
  • Collaborate with our nursing team to ensure seamless care coordination and communication.
  • Continuously assess and revise patient care plans to ensure optimal outcomes.
Requirements
  • Active and current license to practice as a professional Physical Therapist in the state of NJ.
  • Previous home healthcare experience is a plus, but we will provide training for the right candidate.
  • Ability to travel to cases as assigned.
  • Excellent assessment skills, strong written and verbal communication, and a calm and reassuring presence.
