We are seeking a skilled and compassionate Pediatric/Maternity Home Care Registered Nurse to join our team at Northwell Health. As a key member of our home care team, you will provide high-quality, patient-centered care to pediatric and maternity patients in the comfort of their own homes.
Job SummaryThe Pediatric/Maternity Home Care Registered Nurse is responsible for delivering comprehensive, age-specific care to patients with complex medical needs. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans that address the unique needs of each patient.
Responsibilities- Conduct thorough patient assessments to identify health status, medical history, and social determinants of health.
- Develop and implement individualized care plans that address the patient's physical, emotional, and social needs.
- Provide education and support to patients and their families to promote health literacy and self-management.
- Collaborate with our interdisciplinary team to coordinate care and services, including medication management, wound care, and equipment needs.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
To be successful in this role, you will need:
- A current license to practice as a Registered Professional Nurse in New York State.
- Graduation from an accredited School of Nursing, with a Bachelor's Degree in Nursing preferred.
- Specialized certifications, such as PALS, may be required depending on the area of assignment.
