Job Title: Licensed Practical Nurse

At Constructive Partnerships Unlimited, we are seeking a skilled Licensed Practical Nurse to join our team in Staten Island, NY.

Job Summary:

The Licensed Practical Nurse will provide comprehensive nursing care, programming services, and activities of daily living to individuals attending our Day Programs under the supervision of a Registered Nurse.

Responsibilities:
  • Work under the direction of the Registered Nurse in providing nursing care, including medication requisition, administration, and record keeping.
  • Closely monitor the condition of individuals attending the Day Program and respond to changes.
  • Communicate with staff regarding all issues and concerns presented.
  • Participate in interdisciplinary team evaluations and implement designed programs by following prescribed methodology.
  • Maintain records and charts for individuals attending the Day Program, including goal progress, temperatures, and changes in behavior.
  • Complete and respond to all requests for information, including incident review and employee-related grievances.
  • Maintain a clean and safe environment.
  • Share responsibility with the R.N. for the wellbeing of individuals attending the Day Program.
  • Attend screening and semi-annual meetings, as well as special team meetings as needed.
Requirements:
  • NYS License.
  • Minimum 2 years nursing experience.
  • Perform all job responsibilities in full compliance with all applicable laws, rules, regulations, policies, and procedures.
