Asbury Communities is seeking a skilled Care Planning Specialist to join our team. In this role, you will be responsible for conceiving and creating care plans based on comprehensive resident assessments. You will schedule PPS/MDS assessments for residents admitted to the nursing care center and lead the assessment coordination process to ensure accurate and timely completion of all nursing sections of the MDS assessments.
Key Responsibilities:
- Conceive and create care plans based on comprehensive resident assessments
- Schedule PPS/MDS assessments for residents admitted to the nursing care center
- Lead the assessment coordination process to ensure accurate and timely completion of all nursing sections of the MDS assessments
- Communicate effectively with the Director of Therapy and Insurance Case Managers to maximize reimbursement
- Lead the MDS completion process by ensuring accurate and timely completion of all sections by responsible disciplines
Requirements:
- Must be a Registered Nurse currently licensed in Maryland, with at least 2 years of experience in a similar role
- Two years of Long Term Care experience
- RAC certification preferred but not required
- Must be able to follow and give accurate written and oral instructions and possess visual acuity to effectively prepare and read reports
- Supervisory and leadership skills with the ability to communicate effectively with staff, residents, and visitors
- Ability to coordinate and organize own work and the work of others to effectively meet multiple job responsibilities and deadlines
- Must have proficient computer skills to ensure accurate data entry / Microsoft Office/PCC experience preferred
