Job Responsibilities
The Ambulatory Care Nurse is a Registered Nurse responsible for providing nursing care management services to assigned populations. Key responsibilities include:
- Collaborating with physicians to identify at-risk patient populations requiring care management services.
- Serving as a patient/family advocate and communication link between the physician, patient, family, and interdisciplinary healthcare team.
- Acting as a liaison between community resources and healthcare services.
- Supporting the organization's mission, values, and management of patient care services.
- Adhering to the ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and Patient & Family Centered Care (PFCC) model.
Requirements
To be successful in this role, candidates must possess:
- A diploma in nursing or associate's degree in nursing.
- Current registration as a Registered Nurse in North Carolina.
- No prior experience is required, but a strong desire to provide exceptional patient care is essential.
Work Schedule
This position requires a regular work schedule, Monday through Thursday, with no weekend or call requirements.
