Responsibilities:

  • Participate in the development of Individualized Service Plans (ISPs) and collaborate with colleagues to ensure seamless care delivery.
Requirements:

  • Positive attitude, flexibility, and ability to work in a team environment.
  • High School diploma or equivalent required; certifications and previous experience preferred.
