About the Role

TGH Home Care powered by VNA of Florida is seeking a skilled Registered Nurse to provide high-quality care services to patients in their homes. As a key member of our team, you will be responsible for delivering professional nursing services, developing and implementing care plans, and ensuring the best possible outcomes for our patients.

Key Responsibilities
  • Provide initial assessments, oversee progress towards goals, and monitor clinical outcomes for patients
  • Develop, implement, and continuously monitor care plans to ensure quality of care for patients
  • Complete clinical documentation, including OASIS evaluations, orders, and documentation of care coordination
  • Work with physicians and discharge planners to promote health and education to patients and caregivers
  • Function as a liaison with community health-related resources, including DME, oxygen, and infusions
  • Interact with patients, family, team members, and other healthcare professionals
  • Participate in ongoing education and seek opportunities for continuous personal and professional development
Requirements
  • Active Registered Nurse License required
  • Minimum of 1-year clinical experience
  • Home Health/HCHB experience
