In this role, you will work as a patient advocate, collaborating with healthcare professionals to develop and implement personalized care plans. Your primary objective will be to restore motion and maximize the quality of life for our patients, ensuring they achieve their optimal functional level and remain safe in their homes.

Key Responsibilities:

  • Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
  • Conduct thorough assessments and evaluations to identify patient needs and goals.
  • Provide skilled therapy services, including speech, language, and swallowing therapy, as ordered by physicians.
  • Consult with physicians regarding treatment changes and progress.
  • Participate in patient care conferences and contribute to the development of comprehensive care plans.
  • Prepare and maintain accurate clinical and progress notes.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.

Requirements:

  • Current licensure in the State of Florida and completion of fingerprinting.
  • Two years of experience in home health care, preferred.
  • Acceptance of the agency's philosophy and goals.
  • Ability to exercise initiative and independent judgment.
