The Clinical Liaison plays a crucial role in facilitating patient admissions to our inpatient rehabilitation facility. This position requires a proactive approach to ensure that patients are placed in a timely manner through comprehensive clinical evaluations.
Key Responsibilities:
- Conduct thorough clinical assessments to guarantee prompt patient placement.
- Work collaboratively with Case Management and the Medical Director to streamline processes.
- Coordinate patient admissions while effectively communicating individual needs.
- Serve as an advocate for our facility within the broader healthcare community.
Qualifications:
- 1-2 years of experience in acute inpatient rehabilitation settings.
- Familiarity with inpatient rehabilitation protocols and procedures.
- Proficient in reviewing medical records to determine rehabilitation necessity.
- Excellent organizational and communication skills are essential.
- Current licensure in the relevant field as required by state regulations.
- BLS Certification is mandatory.
- CARE Tool Credentialing is preferred when applicable.
Work Schedule: This position requires availability on weekends, specifically Saturday and Sunday, with 8-hour shifts.
