Delivers safe and effective family-centered nutritional care to patients from infancy to adulthood with diverse medical conditions.

Evaluates diagnoses and formulates nutritional care strategies for patients in accordance with established dietetic standards and regulations.

Acts as a resource for colleagues to facilitate problem-solving, engages in shared governance and evidence-based practices, and begins to cultivate clinical leadership abilities in designated areas.

Description

Assesses diagnoses and designs and executes nutritional care plans for patients using established dietetic standards.

Conducts nutritional evaluations and collaborates with the medical team to ensure optimal nutrition care for patients.

Identifies nutrition-related challenges and seeks assistance/referrals for complex issues. Issues therapeutic diet orders within the scope of practice.

Monitors patients' progress.

Communicates effectively with patients, families, and all medical personnel. Provides relevant information (issues, service requests, improvement suggestions, etc.) to management.


Educates and guides patients, families, caregivers, and staff on dietary practices, feeding protocols, or other nutritional insights prior to discharge or during outpatient consultations.

Functions as part of a multidisciplinary medical team. Collaborates with nutritional services to ensure the availability of necessary food items and procures formula from appropriate sources. Works with dietetic interns and offers peer reviews of nutritional assessments and diet orders.

Documents all relevant nutrition information in patients' medical records in accordance with policy. Records daily charges for patients seen, procedures and activities performed, and time allocated for each.

Participates in nutritional case studies, departmental training, and other educational opportunities, documenting continuing education credits in CDR format.

Actively engages in Medical Team rounds.

Performs additional duties as assigned.
