Job Summary

We are seeking a dedicated and compassionate Mental Health Social Worker to join our team. In this role, you will provide essential social work services to patients at our Community Mental Health facility. You will play a vital part in supporting individuals facing social, emotional, and related challenges.

Responsibilities

  • Conduct comprehensive assessments and diagnoses of patients with mental health concerns.
  • Develop and implement individualized treatment plans tailored to each patient's needs.
  • Provide individual, group, and family therapy sessions.
  • Collaborate with a multidisciplinary team of professionals, including psychiatrists, nurses, and case managers.
  • Connect patients with community resources and support services.
  • Maintain accurate and detailed patient records.
  • Participate in emergency on-call services as required.

Qualifications

  • Master's Degree in Social Work (MSW) from an accredited program approved by the New York Department of Education.
  • Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or Licensed Marriage and Family Therapist (LMFT).
  • Strong understanding of social work principles, practices, and ethical guidelines.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work effectively both independently and as part of a team.
