LPN Job Description

**Job Summary:**

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare. As an LPN, you will be responsible for providing high-quality patient care in post-acute facilities in the Warwick, RI area.

Key Responsibilities:

  • Provide direct patient care in accordance with resident care plans and facility policies.
  • Monitor and track the wellbeing of residents, including physical and mental health, and vital signs.
  • Administer and verify medications, and monitor their effects.
  • Review and update resident charts and medical records.
  • Assist with maintaining a safe and comfortable facility environment.
  • Participate in IV set up, immunizations, and phlebotomy procedures.
  • Help with the orientation and training of medical staff.

Requirements:

  • Current Rhode Island state license to practice as a Licensed Practical Nurse (LPN).
  • Completion of an approved Licensed Practical Nurse (LPN) educational program.
  • High school diploma (or GED/equivalent).
  • Minimum of 6 months of experience (LPN or related role).
  • Ability to lift a minimum of 50 lbs.
