Care Coordinator

Lifescape Community Services is seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in assessing and authorizing Community Care Program services, as well as providing ongoing case management services to our clients.
Key Responsibilities- Assess and authorize Community Care Program services for eligible participants
- Provide ongoing case management services to both CCP participants and Title III (Non-CCP) Case Management participants
- Complete comprehensive and redetermination assessments to make eligibility determinations
- Develop and implement Person-Centered Plans of Care (PoC) and Participant Agreements (PA)
- Collaborate with healthcare providers, family members, and community partners to ensure seamless service delivery
- Maintain accurate and detailed case records, including documentation of follow-up, reassessment, and termination
- Participate in the appeals process and other IDoA matters as instructed by the program supervisor
- Comply with all Critical Event Reporting requirements and billing policies and procedures
- Must be 21 years of age
- Must meet the educational and experience requirements designated by Illinois Administrative Code Title 89 Section
- Must have a valid driver's license, clean driving record, and own transportation with current insurance coverage
- Must be able to work and communicate effectively with older persons, persons with sight and hearing impairments, and persons with a variety of economic, social, and cultural backgrounds
- Must be able to perform essential functions of the position
The Care Coordinator will work in a fast-paced, dynamic environment, requiring strong organizational and communication skills. The position will involve working with a diverse range of clients, including older adults, persons with disabilities, and their families. The Care Coordinator will be required to work nights and weekends, as needed.
