As a key member of our staff, you will play a vital role in providing mental health, substance abuse, vocational, and rehabilitative services to adult individuals with persistent and severe mental illness.

Key Responsibilities:

  • Provide mental health and substance abuse services to individuals in a day program setting.
  • Conduct vocational and rehabilitation assessments and develop individualized plans.
  • Coordinate case management services and provide follow-along support to individuals and their families.
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support.
  • Develop and maintain relationships with community resources and providers to ensure seamless care coordination.

Requirements:

  • Certified Psychiatric Rehabilitation Specialist (CPRS) or licensed mental health professional (LPC, LCSW, LMSW, LMHC).
  • Strong knowledge of mental health and substance abuse treatment principles and practices.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced, dynamic environment.
  • Strong problem-solving and critical thinking skills.
