Outreach Ambassador

We are seeking a highly motivated and compassionate Outreach Ambassador to join our team at BoldAge PACE. As a key member of our outreach team, you will be responsible for generating referrals into our PACE program and educating the community through community outreach, networking, and marketing activities.

• Develop and cultivate relationships with referral partners, healthcare providers, and senior service providers to promote our PACE program.
• Participate in community groups, present at community events, and engage in marketing activities to raise awareness about our services.
• Collaborate with the interdisciplinary team to facilitate the assessment and eligibility determination processes.
• Identify quality referrals and respond timely to all inquiries and commitments.

• Bachelor's degree in healthcare administration, business, communications, marketing, or a related human services field.
• Minimum 1 year experience working with the elderly, preferably in community outreach and engagement.
• Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
• Knowledge of managed care environments and community resources.
