Key Responsibilities:
- Conduct thorough medical history assessments, diagnoses, and treatment plans for patients in their homes or skilled facilities.
- Collaborate with primary care physicians, case managers, social workers, family members, and other healthcare professionals to ensure seamless care coordination.
- Provide education and guidance to patients and their families on healthy lifestyle choices and disease management.
- Participate in quality improvement initiatives to enhance patient outcomes and experience.
Requirements:
- A Master's degree in Nursing or a related field, or equivalent experience as a licensed nurse practitioner.
- Current RN and NP licensure in the state of practice, including prescription authority.
- Board certification by the American Association of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or American Association of Critical-Care Nurses (AACN).
- A minimum of one year of experience as a nurse practitioner, preferably in home health or gerontology.
