Job Overview

We are seeking a skilled RN/LPN to join our team as a Clinical Assessment Specialist. As a member of our Community Care Team, you will be responsible for conducting comprehensive assessments of individuals receiving MassHealth Personal Care Attendant Services.

In this role, you will have the opportunity to make a meaningful impact on the lives of older adults, people with disabilities, and caregivers in Hampshire and Hampden Counties.

About the Role

The Clinical Assessment Specialist will perform clinical assessments in-person at the consumer's home, utilizing excellent assessment and evaluation skills. You will work collaboratively with Occupational Therapists, Functional Skills Trainers, and the PCA Program Director to ensure consistency and compliance of all evaluations.

Key responsibilities include:

  • Assessing PCA Consumers' functional limitations and identifying disabling diagnoses and other medical conditions.
  • Completing MassHealth Applications and Evaluations for PCA Services in compliance with MassHealth regulations.
  • Collaborating with interdisciplinary teams to provide high-quality care.
Requirements

To be successful in this role, you must have:

  • A current Massachusetts RN/LPN License.
  • At least one (1) year of clinical and/or nursing experience in a community or long-term care setting.
  • Excellent assessment and evaluation skills.
  • Computer proficiency to process consumer records in electronic databases.
  • Ability to work independently and as part of an interdisciplinary team.
Compensation and Benefits

As a Clinical Assessment Specialist, you can expect a competitive salary range of $32-$34 per hour, plus a 6% pay differential for bilingual skills. Enjoy a $5,000 sign-on bonus payable in two equal installments after 3 and 6 months of employment.
