Nephrology Care Specialist

Nephrology Associates of is seeking a highly skilled Nurse Practitioner to join our clinical team. As a key member of our team, you will provide high-quality patient care under the supervision of a physician, within the scope of your training and education.
Key Responsibilities:
- Conduct comprehensive physical assessments and diagnose, treat, and manage acute and chronic health problems.
- Order laboratory and radiology tests and interpret results with treatment management.
- Prescribe medications, including controlled substances within the scope of practice.
- Provide patient education, health maintenance, and promotion.
Clinical Support Functions:
- Collect data to support patient assessment, including vital signs, social history, chief complaint, and current concerns.
- Alert physicians to any patient complaints, especially those that are life-threatening.
- Support clinical functions to prepare patients for examination, treatment, or procedure.
- Call pharmacy refills and complete patient callbacks according to office protocol and as directed by a physician.
- Document all calls, correspondence, and encounters with patients within established guidelines.
Direct Patient-Care Functions:
- Assess patients' conditions, learning needs, and provide education as directed by regarding treatment, special diets, medication, follow-up diagnostic tests, and healthy lifestyle information.
- Round on patients in a dialysis setting on designated schedules.
- Administer prescribed medications and treatments as directed by physicians in accordance with nursing standards.
- Perform procedures and treatments within the scope of your training and experience, as ordered by physicians following established protocols and appropriately document outcomes in patient records.
- Triage emergency and non-emergency phone calls with appropriate destinations and document according to protocols.
- Follow established protocols in obtaining, preparing, and transporting specimens for diagnostic tests as ordered by physicians and using universal precaution guidelines.
- Observe, record, and report patients' conditions and reactions to drugs and treatments to physicians.
- Assist in coordinating patient transfers to hospitals (direct admissions).
- Follow established protocols to report and document diagnostic tests as directed by physicians, within the scope of your training and experience.
- Review patient records and other pertinent information and take appropriate action within nursing standards and office protocols.
- Administer medications when prescribed by physicians and document appropriately in patient records.
- Document and report clinical information or complaints taken over the phone to physicians in an appropriate manner within established timeframes.
Requirements:
- Must have 3+ years' experience as a practicing Certified Nurse Practitioner. Prior experience in Nephrology, cardiology, ICU, or CCU is preferred.
- Master's Degree in Nursing or Science of Nursing.
- Current Nurse Practitioner Certification from AANP or ANCC.
- Current Nursing License as Registered Nurse and as a Certified Nurse Practitioner.
- ACLS, BLS, and DEA.
- Valid driver's license if the position requires provision of services at multiple medical practices.
- Excellent time management skills and ability to multi-task and prioritize work in a fast-paced environment.
- Must be self-starters who can work independently as well as in a highly collaborative, fast-paced team environment.
- Patient-focused with strong communication and organizational skills.
- Renal experience is highly preferred.
