BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in Belmont, Massachusetts.
About the Role:We are looking for a compassionate and experienced Physical Therapist to provide exceptional rehabilitative therapy to geriatric patients in Senior Living Communities. As a member of our multidisciplinary healthcare team, you will work closely with nursing staff to develop and implement individualized care plans, ensuring our clients receive the medical care required to maintain their independence.
Responsibilities:- Perform diagnostic tests and measurements to assess client mobility, strength, and balance.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
- Continuously assess and revise care plans, participating in multidisciplinary care planning as needed.
- Educate and instruct clients, family members, or caregivers on rehabilitative care and activities necessary for independent living.
- Accurately document observations, interventions, and evaluations related to client care management and services provided.
