Job SummaryWe are seeking a skilled Palliative Care Nurse to join our team. As a Palliative Care Nurse, you will work with patients, their families, and healthcare professionals to provide and coordinate high-quality end-of-life care. You will conduct in-person and telephonic assessments, provide education to patients and families, and work to ensure patients are comfortable in their home environment.
Key Responsibilities- Conduct in-person and telephonic RN assessments and screenings
- Provide education to patients and families on disease process, self-care techniques, end-of-life care, medications, nutrition, and dietary needs
- Consult with and educate patients/families regarding disease process, self-care techniques, end-of-life care, medications, nutrition, and dietary needs
- Communicate findings to the nurse practitioner and/or PCP as needed, but no less than bi-weekly
- Re-assess effectiveness of care plan on a regular basis and modify as needed
- Nursing experience in palliative care, oncology, hospice, or geriatric care preferred
- Additional experience in hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing
- Fluent competence in computer programs Microsoft Outlook, Microsoft Word, and Microsoft Excel
- Fluent in electronic medical record documentation
- Strong compassionate phone skills
- Strives for independence and sound critical decision-making regarding the delivery of patient care
- Good time management skills
