Staff Nurse

The Staff Nurse is a vital member of the patient-centered medical home team at HopeHealth, Inc. This role involves managing individualized patient care, promoting and restoring health through the nursing process, and collaborating with providers and multidisciplinary team members.
Key Responsibilities- Assess patient needs based on data collected through history, observation, physical exam, telephone triage, and analysis of diagnostic data.
- Perform direct patient care, including triage, assessment, procedures, treatment, patient education, and follow-up instructions.
- Assist patients and providers in obtaining medical prior authorizations when required.
- Verify patient understanding of the purpose of the visit.
- Provide preventative care information matching the patient's age, gender, and health status.
- Maintain knowledge of principles and skills needed for practical nursing to provide patient care and treatment.
- Knowledge of examination, diagnostic, and treatment room procedures.
- Ability to interpret, adapt, and apply guidelines and procedures.
- Basic Critical Care and Chronic Illness Management.
- Participate in training opportunities to enhance skills and knowledge.
- Contribute to office efficiency and encounter volumes.
- Ability to understand and administer vaccines and immunizations to pediatric and/or adult patients.
- Responsible for daily clinical workflow, including referral management, telephone encounters, lab results, medication refills, lab Q&A, and compliance with sample medication logs.
- Willingness to learn about AIDS Service Organizations and Community Health Centers.
- Assist in training new nurses to the organization in standard working procedures.
- Diploma in Nursing or Associate's degree in Nursing.
- Licensed by the State Board of Nursing.
- BLS certification required.
- Two years of medical office experience preferred.
- Excellent oral and written communication skills.
- Ability to collaborate with others to improve patient experience and address needs.
- Knowledge of policies, procedures, regulations, and protocols.
- Knowledge of databases, including EMR systems.
- Bilingual in English/Spanish preferred.
- Familiarity with medication names and usage.
- Medical terminology.
- At least two years of experience in a physician's office.
- Excellent interpersonal skills.
- Detailed-oriented.
- Ability to multitask.
- Compassion for patients.
- Ability to administer a variety of treatments and medications as directed.
- Capable of adapting to computer software.
- Adequate hearing to perform job duties in person and over the telephone.
- Ability to communicate clearly to patients in person and over the telephone.
- Visual acuity to perform job duties, including reading information from printed sources and computer screens.
- Occasional lifting and carrying items weighing up to 10 pounds unassisted in normal daily activities and up to 30 pounds for other required work activities.
- Frequent bending, reaching, and repetitive hand movements, especially if keyboarding and writing.
- Standing, walking, squatting, and sitting, with some lifting, pushing, and pulling exerted regularly throughout a regular work shift.
