CenterWell Physician-Woodstock

We are seeking a skilled and compassionate Physician to join our team at CenterWell Senior Primary Care. As a key member of our integrated care team, you will provide high-quality, patient-centered care to seniors in our community.

• Provide proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
• Work collaboratively with our integrated care team, including nurses, behavioral health specialists, and other healthcare professionals to deliver comprehensive care.
• Develop and implement personalized care plans that address the unique needs and goals of each patient.
• Stay up-to-date with the latest medical research and best practices to provide evidence-based care.
• Participate in quality improvement initiatives to enhance patient outcomes and experience.

• Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
• Graduate of an accredited MD or DO program.
• 5 or more years of technical experience in a healthcare setting.
• Licensure requirements of the state of jurisdiction.
