**Job Description**

We are seeking an experienced Registered Nurse to join our team. As a Home Care Nurse, you will provide high-quality patient care, conducting thorough assessments and developing effective care plans to ensure optimal patient outcomes.

In this role, you will work collaboratively with other disciplines to meet patient-specific needs and maximize patient safety and outcomes. You will also be responsible for:

  • Conducting thorough patient assessments and creating effective care plans.
  • Developing and implementing plans of care that address patient-specific needs, preferences, and clinical conditions.
  • Communicating patient information and changes in status to physicians and documenting in a timely manner.
  • Reconciling medications accurately at SOC, ROC, and transfer, and incorporating changes into the plan of care.
  • Managing medical supplies according to allowable guidelines and collaborating with the WOCN to manage wound supplies.
  • Meeting point of care standards for data submission, including closed and sync'd encounters.
  • Collaborating with other disciplines to meet patient-specific needs and maximize patient safety and outcomes.
  • Providing patient education in the patient's preferred language and format, and evaluating ongoing to meet patient needs.
  • Initiating discharge planning at Start of Care and evaluating ongoing for optimal patient safety and outcomes.

**Salary**: $80,000 - $120,000 per year, depending on experience.

**Requirements**

To be considered for this role, you must have:

  • A graduate degree from an accredited School of Nursing.
  • A current Massachusetts license as a Registered Nurse.
  • A minimum of one year of previous nursing experience in acute care or sub-acute care setting.
  • Prior home care experience is preferred.
  • Ability to work with various computer software and knowledge of hand-held patient care computer devices is preferred.
  • Strong interpersonal and customer service skills and ability to work with a diverse population.
  • Ability to communicate effectively in writing, verbally, and electronically.
  • Ability to work as a member of an interdisciplinary team of health care providers.
  • Ability to organize and prioritize work and adapt to changing situations.
  • Ability to work independently, be self-directed, and adapt to unpredictable circumstances.
