Job Summary

We are seeking an experienced Home Health Licensed Practical Nurse to join our team for a 13-week travel nursing assignment in a dynamic and supportive environment. As a Home Health LPN, you will have the opportunity to work with a diverse range of patients and families, providing high-quality care and support in the comfort of their own homes.

Responsibilities

  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Conduct thorough assessments of patients' physical and emotional needs, and make recommendations for care and support.
  • Document patient progress and outcomes in a timely and accurate manner.

Requirements

  • Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
  • Minimum of 1 year of experience in home health care or a related field.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to lift, push, and pull up to 25 pounds.
