The Clinical Liaison is responsible for facilitating timely and appropriate patient placements through thorough clinical evaluations and assessments. This role requires collaboration with hospital Case Management and the Rehabilitation Medical Director to ensure seamless transitions for patients.
Key Responsibilities:
- Collaborate with the Inpatient Rehabilitation Facility (IRF) staff to manage admissions and address any specific patient requirements.
- Gain a comprehensive understanding of IRF operations and quality metrics, positioning oneself as a key advocate for the IRF within healthcare networks and the broader community.
Qualifications:
- Preferred 1-2 years of experience in an acute inpatient rehabilitation environment.
- Strong familiarity with inpatient rehabilitation practices.
Essential Skills:
- Proficient in conducting detailed medical record reviews to assess rehabilitation medical necessity.
- Excellent organizational abilities with a focus on results.
- Self-motivated and capable of managing multiple tasks effectively.
- Strong communication skills to engage with diverse audiences, including both professionals and laypersons.
- Ability to adhere to established timelines and deliver exceptional customer service to both internal and external stakeholders.
- Capable of working independently and proficient in using computers and electronic data entry systems.
Licensure and Certifications:
1. Current State of Florida license as a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Therapist.
2. BLS Certification.
3. CARE Tool Credentialing (when available in UDS).
Work Schedule:
Saturday and Sunday - 8-hour shifts.
