Job Summary

We are seeking a highly skilled and dedicated individual to join our team as a Health Planner at the City of Amarillo, Texas. This is an exciting opportunity to work in a dynamic environment and contribute to the development and implementation of health programs that promote the well-being of our community.

Key Responsibilities
  • Coordinate activities related to the Community Mental Health Grant, focusing on mental health and suicide prevention.
  • Work with law enforcement agencies, volunteers, survivors of suicide, and partner organizations to meet grant deliverables.
  • Develop and implement health education and health planning efforts for the CMHG program.
  • Ensure compliance with all reporting and DSHS required participation guidelines set in the grant.
  • Coordinate LOSS Team volunteers, training, debriefing, and preparation.
  • Provide case management services for clients and document in electronic medical record systems.
  • Participate in the Amarillo Area Suicide Review Team and communicate effectively with colleagues and stakeholders.
  • Monitor and report on CMHG performance measures and ensure timely implementation of projects.
Requirements
  • Bachelor's degree in Social Work, Nursing, Education, or other health-related field.
  • Valid Texas Driver's License and reliable transportation with proof of current vehicle insurance.
  • Knowledge of area of specialty, anatomy and physiology, medications, and disease processes.
  • Strong verbal and written communication skills and ability to work with a culturally diverse population.
  • Ability to handle confidential information and travel out of town for trainings and seminars.
