Job Summary:
The Care Coordinator RN plays a vital role in facilitating the plan for inpatient and outpatient care by using a collaborative interdisciplinary approach.
This role requires a skilled nurse to focus on anticipation, integration, and coordination of clinical care and discharge planning services to meet the needs of patients and optimize clinical and resource outcomes.
Responsibilities:
- Collaborate with the interdisciplinary team to assess patient needs and develop the plan of care.
- Act as an advocate for the patient, physician, and hospital relative to compliance with federal, state, and third-party utilization regulatory requirements.
- Work closely with the care team to ensure seamless transitions and effective communication.
Requirements:
- 3 years of experience in a nursing-related area, with 3 years of clinical experience in case management, utilization management, or home/community health.
- Graduate nursing degree required, with a Bachelor's degree in nursing preferred.
- Registered Nurse licensure required.
Work Schedule:
8:00 am - 4:30 pm, Monday - Friday
