Job Summary:The Clinical Pharmacist will be responsible for providing pharmaceutical services, including medication therapy management, drug information, and clinical research. You will work closely with healthcare teams to ensure patients receive the best possible care.
Key Responsibilities:- Provide medication therapy management and counseling to patients
- Develop and implement clinical pharmacy services, including medication use evaluations and formulary management
- Collaborate with healthcare teams to optimize patient care and outcomes
- Conduct clinical research and participate in quality improvement initiatives
- Stay up-to-date with the latest developments in pharmacy practice and apply this knowledge to improve patient care
- Doctor of Pharmacy (PharmD) degree from an accredited institution
- Current licensure as a Registered Pharmacist (RPH) in the state of California
- 2+ years of experience in clinical pharmacy practice
- Strong communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize multiple tasks
