About the Role

We are seeking a skilled Direct Care Counselor to join our team at Above It All LLC. As a Direct Care Counselor, you will be working closely with clients to provide support with entitlements, mental health services, and coordination of care.

Responsibilities:
  1. Client Sessions: Conduct weekly sessions with consumers implementing rehabilitation activities outlined in their Individualized Rehabilitation Plans.
  2. Documentation: Document all attempted and completed encounters with consumers, families, and other agencies within our Electronic Health Record system.
  3. Communication: Communicate and plan services with consumers and their supports.
  4. Coordination: Collaborate with outside agencies to arrange community services and coordinate care.
  5. Referrals: Make appropriate referrals.
  6. Reporting: Report issues and concerns to leadership and plan interventions.
Qualifications:
  • An associate degree in a health-related field or 30 hours of college credit toward a Bachelors degree in a health-related field.
  • 1 year of work experience in a supervised mental health setting.
  • A valid driver's license with an acceptable driving record and personal transportation.

Salary range: $45,000 - $60,000 per year, based on experience and qualifications.
