Key Responsibilities:
- Conduct comprehensive evaluations to assess the social, emotional, and support needs of patients and their families.
- Develop measurable objectives in conjunction with the rehab team to ensure effective treatment plans.
- Collaborate with healthcare professionals to understand the significant social and emotional factors related to patients' health problems.
- Provide instructive counseling to patients and families on treating and coping with social and emotional responses connected with illnesses.
- Participate in care coordination activities and serve as a resource to other health team members in identifying and resolving patient needs.
- Assist patients and families in utilizing community resources to achieve measurable objectives.
Requirements:
- Master's Degree in Clinical Social Work (LCSW)
- Current Licensed Clinical Social Worker Licensure in the state of practice
- Minimum of one year experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury, or acute diagnosis
- CPR certification required
