The Clinical Social Worker, MSW will play a critical role in the Emergency Department, working closely with the healthcare team to provide high-quality patient care. Key Responsibilities- Patient Assessment and Planning: Conduct thorough assessments of patients and their families to identify social, emotional, and economic factors that may impact their care.
- Treatment Planning and Implementation: Develop and implement individualized treatment plans to address the unique needs of each patient.
- Community Resource Utilization: Identify and connect patients with community resources to support their recovery and well-being.
- Collaboration and Communication: Work closely with the healthcare team to ensure seamless communication and coordination of care.
- Discharge Planning: Collaborate with the RN Case Manager to develop discharge plans that meet the needs of patients and their families.
- Master's Degree in Social Work: A Master's degree in Social Work (MSW) is required for this position.
- Continuing Education: Evidence of continuing education and professional development is essential.
- Regulatory Knowledge: Familiarity with regulatory requirements related to reporting of child/elder abuse, substance use/abuse, child protective custody, adoptions, and other legal requirements.
- Technical Skills: Proficiency with main-frame and personal computers is necessary.
